This plan covers you and your family even while overseas whether in need of emergency or non-emergency medical care.

COVERAGE

If you are without health insurance or wish to add to your existing health care benefits, you need health coverage at a price that you can afford. Sagicor's line of Independent Health Coverage Plans offers you a range of benefit options to satisfy your personal and family health needs and budget.

When you select the Family Coverage, you do not have to worry about the health care needs of your spouse, whether married or un-married, and your children. Under an individual health plan, the health care benefits apply only to you.

SPECIAL BENEFITS

• Conversion Privilege

  If you are on a Sagicor Group Plan and terminate your employment, you can continue your coverage, by transferring to one of these plans within thirty (30) days of the termination of your employment. You would not be required to pass a medical and there is no waiting period for pre-existing conditions.

• Increase Your Coverage

  You may increase your coverage at any time. However, you may need to provide new evidence of insurability.

• Coordination of Benefits

  The Supreme Health, Executive Health and Executive Health Plus Health Plans allow for the coordination of benefits where you, or your dependents are already covered under more than one health plan.

ENROLMENT

1. Apply online, call or visit the Sagicor office nearest you. One of our knowledgeable Sales Representatives will assist you in selecting the plan that best suits your needs.

2. Submit your completed application form and premium to Sagicor Life Jamaica Limited. This will be medically underwritten for approval. Once you qualify, your coverage will continue, regardless of your future medical condition.

3. Once your premiums have been accepted at the office of Sagicor Life Jamaica Limited you are eligible for immediate benefits. You will be able to access health care services, except where waiting periods apply using your Sagicor Membership Card.

PREMIUMS

Your premium should be paid on the due date, or within thirty (30) days of that date. Avoid policy cancellations by ensuring that your premiums are paid on time each due date.

You have two (2) premium payment options:

• Annually
  
• Semi-annually
  

Monthly premiums are allowed only if the premium is being paid through your company via salary deduction under the Individual Health Coverage Plans. If you opt to pay monthly premiums yourself you will be required to pay for your medical services and submit claims to and for reimbursement by Sagicor Life Jamaica Limited.

Premiums paid on an annual basis will receive a discount.

WAITING PERIODS

Waiting periods apply in respect of the following conditions:

• Pregnancy
  Nine (9) months for maternity benefits and benefits for any condition related to pregnancy.
  
  
• Pre-existing medical and surgical conditions
  Six (6) months for such conditions, whether known or unknown.
  
  
• Hospitalization and services which go into Major Medical
  Six (6) months.
  
  
• Change of spouse
  This is done only on the anniversary date of the contract or where marriage has taken place.

HOW TO ACCESS BENEFITS

To access benefits, you must show your Sagicor Membership Card to the provider - doctor, pharmacy, laboratory and diagnostic service provider.

CLAIMS

You must submit claims within 90 days or service. Sagicor reserves the right to deny payment of claims submitted after 90 days.

You must affix your membership number, and correct name and address as failure to do this will result in your claim not being processed.

You should also state your Sagicor number on all correspondence you send to Sagicor or if you call the office with a query.
